• 12 months of monthly payment.
• Your card will automatically be charged each month.
• Membership begins on the date of enrollment.

• Rewards cannot be combined with other special offers or promotions and cannot be redeemed for cash.
• Membership card benefits are only valid for the cardholder and are not transferable.
Monthly services not redeemed do not roll over into the next month.
• Pre-purchase of treatments before program expiration is prohibited.
• There are no refunds for paid membership fees or new treatments.
• Memberships are automatically renewed unless canceled by either party with a 30 day notice.
• ANNUAL ONLY: The contract is annual and will automatically renew after the first 12 months.
• Redemption of services may begin upon completion of authorization of the credit card and/or any other payment method used.
